Outreach is social care provider with over 28 years experience in caring for people in their own homes.
We are looking for honest and caring people to join our existing team. This role requires you to have a full enhanced police check this is paid for by the company.
The role is a mixture of office and community based covering the Southport area.
The team leader role requires you to work with the care manager to ensure high standards of care are delivered to our clients at all times. You will monitor the performance of care staff, and offer help and support to them when needed, to ensure high standards are maintained. You will work alongside staff delivering care to clients when needed as part of the team leader role.
Main Duties:
- Comply fully with all guidelines and requirements detailed in the Company Handbook and Procedure Manual.
- Always represent the Company professionally, support staff in meeting performance standards, and ensure that everyone contributes to the Organisation’s efficiency and reputation.
- Assist with reviewing care plans and risk assessments as directed by the Care Manager.
- Visit clients quarterley to conduct Quality Assurance questionnaires.
- For clients needing medication assistance, make sure Medication Administration Records are present andup to date in their homes.
- Submit monthly reports, timesheets, and carer documentation to the Care Manager at weekly meetings.
- Address staff and client concerns, seek advice when needed, record all details and outcomes on digital system, and notify the Care Manager.
- Train new members of the care team.
- Carry out bi-annual observation of care staff practices.
- Travel within your area to perform Team Leader responsibilities for clients.
- Cover the manager on-call phone on a rota basis.
- keep all digital records updated.
Pay:
Base - £14 per hour
Weekends / Evenings - £15 per hour
Travel time and Mileage at 45p
On call duties
paid at £37.50 per evening monday - thursday
Weekend - 5pm Friday to 9am Monday paid at £187.50
Requirements:
- Valid driving licence and own car
- Female and male applicants are welcome
- Minimum 1 year experience in a social care environment
- Willing to undergo an NVQ level 3 in Health & Social Care
