Are you a passionate and experienced Practice Nurse seeking a challenging and rewarding role within a dynamic primary care setting? We are looking for a dedicated individual to join our Enhanced Access Service team as a Practice Nurse.
Main duties of the job
- Identify, manage and support patients at risk of developing long term conditions, preventing adverse effects on the patients health.
- Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
- Request pathology services as required.
- Provide chronic disease clinics, delivering patient care as necessary, referring patients into secondary /specialist care as required.
- Maintain accurate clinical records
- Ensure read codes are used effectively.
- Deliver well woman clinics.
- Deliver well man clinics.
- Carry out cytology screening including smears.
- Prioritise health issues and intervene appropriately.
- Recognise, assess and refer patients presenting with mental health needs.
- Support patients in the use of their prescribed medicines or over the counter medicines (within scope of practice).
- Support the clinical team with all safeguarding matters in accordance with local and national policies.
- Deliver opportunistic health promotion where appropriate.

Qualifications
- NMC Registration.
Experience
- Experience of working within a team working autonomously in a
- Education.
- Evidence of further development of own clinical practice.
- Understand the importance of evidence-based practice
- ability to record accurate clinical notes.
- Ability to work within own scope of practice and understanding when
- to refer to GPs.
- Working within a community setting
- Experience of General Practice/ Primary Care Experience.
- Broad knowledge of clinical governance
- Awareness of issues within the wider health arena
- Knowledge of health promotion strategies.
- Driving Licence access to independent travel.
