Are you a passionate and experienced Practice Nurse seeking a challenging and rewarding role within a dynamic primary care setting? We are looking for a dedicated individual to join our Enhanced Access Service team as a Practice Nurse.
Main duties of the job:
- Identify, manage and support patients at risk of developing long term conditions, preventing adverse effects on the patients' health.
- Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
- Request pathology services as required.
- Provide chronic disease clinics, delivering patient care as necessary, referring patients into secondary/specialist care as required.
- Maintain accurate clinical records.
- Ensure read codes are used effectively.
- Deliver well woman clinics.
- Deliver well man clinics.
- Carry out cytology screening including smears.
- Prioritise health issues and intervene appropriately.
- Recognise, assess and refer patients presenting with mental health needs.
- Support patients in the use of their prescribed medicines or over the counter medicines (within scope of practice).
- Support the clinical team with all safeguarding matters in accordance with local and national policies.
- Deliver opportunistic health promotion where appropriate.
About us:
Employment will be with OWLS CIC Ltd West Lancashire GP Federation, as a central function to the GP Practices and Primary Care Network (PCN) members. OWLS CIC is a small GP owned and led not-for-profit primary care organisation, run by GPs and health professionals. OWLS was founded in the 90s by a small group of General Practitioners, with the main aim of ensuring high quality out of hours services. In 2017 it transitioned to become a GP Federation to support and provide services for its practices and to offer a vehicle that local GPs had an opportunity to bid for, and provide, innovative primary care services in their local area. We are a not-for-profit organisation, which means that all the money we generate through service contracts is invested back into providing patient care.

Person Specification:
- Qualifications: NMC Registration.
- Experience: Experience of working within a team working autonomously in a community setting.
- Education: Evidence of further development of own clinical practice.
- Understand the importance of evidence-based practice and ability to record accurate clinical notes.
- Ability to work within own scope of practice and understanding when to refer to GPs.
- Experience of General Practice/Primary Care.
- Broad knowledge of clinical governance.
- Awareness of issues within the wider health arena.
- Knowledge of health promotion strategies.
- Driving Licence access to independent travel.
