Position Summary: Case Managers work in collaboration with the clinical team to identify, coordinate, and secure community-based resources to support the Adolescent client’s ongoing treatment and recovery. The case manager also facilitates the admission and discharge process, coordinates outpatient care, and maintains up-to-date knowledge of relevant community resources and the procedures for accessing services from external providers and social agencies.

Schedule: Part Time: Monday- Friday 8:00 am-1:00 pm

Location: RTP Child, Adolescent, and Adult Inpatient and Residential Treatment Center (Stirrup Creek Drive, Durham, NC)

Compensation Disclosure: Starting hourly range between $22-27 per hour. Final Compensation offered will be within pay range based on qualifications/experience met for the position.

How Case Managers Empower Recovery:

  • Facilitates the admission process by partnering with the admissions team and site staff to arrange for client needs.
  • Assesses post discharge needs and communicates client discharge needs to the treatment team.
  • May partner with families or other supports to coordinate discharge plans.
  • Partners with Accanto staff to coordinate internal steps to lower levels of care and with external step-down providers and referring providers to coordinate client care including treatment appointments, lodging, and other community resources.
  • Documents in the electronic health record in accordance with Accanto policies and other applicable regulatory standards.
  • Maintains working knowledge of community resources and referral procedures. Networks with other organizations to foster referral relationships.

Qualifications:

  • Bachelor’s degree in mental health related field required, Licensed Social Worker preferred.
  • Mental health case management experience preferred.
  • Must have a valid driver’s license and be authorized to drive.
