We are seeking a Social Prescriber to join our growing multi-disciplinary team at OurHealth Partnership, working within the Moseley, Billesley and Yardley Wood Primary Care Network (MOBY PCN). Please note whilst our Head Office is in Cotteridge, this is not where the role is based, you will be required to work within any of the listed practices within the PCN.

Main duties of the job

  • The role is to deliver a Social Prescribing Service to the practices across the PCN and neighbourhood.
  • To build working knowledge of services in local areas in preparation of engaging with patients and supporting/signposting to achieve best patient outcomes.
  • To play a lead role in the integrated neighbourhood team attending weekly MDT meetings.
  • To implement patient referrals from participating GPs and other primary care health professionals, providing holistic assessments, co‑designing a social prescription to improve health and well‑being outcomes for individuals with a longer term outcome of reducing the number of clinical/medical interventions required.

About us

Our Health Partnership was set up by local GPs who are passionate about providing high quality primary care and using their time and skills effectively to benefit patients. We are currently a GP partnership of 29 practices with 38 surgeries, serving around 280,000 patients in Birmingham, Wolverhampton, and Shropshire. The partnership offers a shared administrative and management structure, cutting down the time doctors have to spend on admin. It opens up economies of scale to get best value from budgets. It has the resources to develop innovative services and effective partnerships with local hospitals and care services. And it can access new funding streams that are only available to large GP organisations.

Job responsibilities

  • To undertake holistic assessments and co‑design Health and Well‑being Plans with individual service users, identifying support needs to ensure maximum engagement in improving health and well‑being.
  • To provide service users with continuity and a coordinated experience of care, remaining point of contact throughout the individual's social prescription.
  • To establish and maintain effective liaison with stakeholders including health, voluntary, social and education resources, attending relevant meetings as necessary.
  • To work in partnership with all voluntary and community organisations to build a comprehensive database of local resources to design and support the Social Prescription Menu.
  • To ensure information on sources of voluntary and community support is up to date at all times to enable effective and accurate signposting and linking of individuals with services.
  • To train and develop GPs and primary care health teams knowledge on how to identify patients suitable for social prescribing service referral on a quarterly basis.
  • Set up and maintain comprehensive data and evaluation systems, including individual health outcome tools.
  • Set up service steering group including representation from all stakeholder groups to support the ongoing development, monitoring and evaluation of the programme.
  • Provide quarterly comprehensive outcome-focused reports detailing the progress of the service.
  • To keep records of your work and adhere to confidentiality, information sharing protocols and provide monitoring information as required.
  • To ensure you have an understanding (appropriate to your role) of, and comply with practice Safeguarding Policies, working closely with both Safeguarding leads and other Care Coordinators to promote and safeguard the welfare of children and vulnerable adults.
  • To comply with OHP Health and Safety Policy, Data Protection Policy and to protect the health, safety and welfare of yourself and others.
  • To work flexibly as required by the service and to take part in OHP/PCN and other organisations meetings and events to promote, support and celebrate the work of the service and the agencies.
  • To be able to evidence: Striving for excellence in everything we do. Having mutual respect for everyone we work with, work for and support through our services.

  • Ability to speak more than one language would be an advantage.
  • Ability to work hours in a flexible way, including occasional evenings and weekends to meet the needs of the service.

Qualifications

  • A health, social care, counselling or other relevant professional or academic qualification.

Skills & Attributes

  • Excellent holistic assessment as well as consultation skills and experience of providing empowering support to adults in a planned and structured way to improve health, recovery and well-being outcomes.
  • Experience of training, supervising, motivating and developing volunteers to ensure a high quality service that meets the needs and promotes positive outcomes for individual service users.
  • Proven skills in collating information and data on community resources and organising these in up-to-date and accessible formats for a range of different service users from various communities.
  • Excellent record keeping skills and the proven ability to write comprehensive reports.
  • IT skills and ability to do own administration using database, PowerPoint and other IT packages.
  • A confident and professional approach to working with a variety of practices.
  • Excellent written, verbal, listening and presentation skills.
  • A proven understanding of safeguarding for children and vulnerable adults and ability to implement relevant policies and procedures.
  • The ability to work autonomously and to plan, prioritise work under pressure and adapt to new models of working.
  • A commitment to equal opportunities and an understanding of the impact on individuals, families and communities health of deprivation.
  • Ability to speak more than one language is an advantage.
