We are looking for a motivated and compassionate practice nurse to join our patient focused and friendly team.
Main duties of the job:
The ideal candidate will be experienced in primary care, long term condition management and health promotion and will play a key role in delivery of high-quality care to patients with chronic and complex health conditions. They will provide advanced clinical care to patients whilst leading in chronic disease management, infection control and Quality Framework Outcomes.
About us:
Our Health Partnership was setup by local GPs who are passionate about providing high quality primary care and using their time and skills effectively to benefit patients. We are currently a GP partnership of 30 practices with 38 surgeries, serving around 280,000 patients in Birmingham, Wolverhampton, and Shropshire.
Job responsibilities:
- Provision of chronic disease management, for patients with Diabetes, COPD, Asthma, Cardiovascular disease and Hypertension
- Domiciliary care and care home cover
- Undertake assessments, reviews and care planning
- Maintain accurate, contemporaneous clinical records
- Work collaboratively with multi-disciplinary teams and General Practitioners
- Participate in quality improvement and audit activities
- Other duties will include health screening, vaccines and injections as appropriate health promotion activities
- Practice Infection Control lead

Person Specification:
- Personal Qualities: Strong communication and interpersonal skills; Ability to work independently and as part of a team; Competent in clinical assessment and patient education; Ability to work effectively in a fast-paced environment, managing own clinical workload.
- Qualifications: Registered Nurse with current NMC registration.
- Experience: Experience working in general practice, primary care or community care; Experience in use of clinical system SystmOne.
- Knowledge and Skills: Diabetes management training; COPD/Respiratory/Spirometry training.
