Salary: £20,007.19 per annum (FTE £25,342.44 per annum)

Hours: 30 per week

Shifts: 07:00am-14:30pm and 14:30pm-22:00pm

About Swann House: Swann House is an Extra Care service situated in the market town of Saxmundham, in the heart of Suffolk's Heritage Coast region. The service is set on a quiet residential road and is less than half a mile from the town centre, which offers a good range of amenities including shops, cafés, public houses and a GP surgery. Saxmundham also benefits from excellent transport links, with a bus stop approximately 200 yards from the service and regular local bus routes connecting the town with Leiston, Aldeburgh and surrounding villages.

Key responsibilities:

  • Provide support with daily living, including personal care, mobility, social activities, mealtimes and medication.
  • Build positive, person-centred relationships and offer emotional support.
  • Use clear communication, listening and observation skills, including recording and reporting in care plans.
  • Support people with a range of health needs, including dementia, mental health conditions and end-of-life care.
  • Maintain an understanding of safeguarding responsibilities and act appropriately.
  • Contribute to person-centred care planning focused on individual needs rather than tasks.

Person Specification:

  • Care Certificate is desirable.
  • QCF Level 2 in Health and Social care or equivalent, is desirable.
  • Full driving licence is desirable.
