Overview
Age Connects Cardiff & the Vale is seeking a dedicated and compassionate Community Liaison Officer to join our Broker Service, working in partnership with the Cardiff Community Resource Teams (CRTs).
The successful candidate will be passionate about making a real difference in the lives of older people, their carers and families.
We are looking for a dynamic individual to join our team.
In this exciting role you will be integrated within the Cardiff Community Resource Team. The role is hands-on, working directly with individuals to help resolve specific housing, social and financial issues. You will provide direct support, empowering people to overcome barriers and access the services they need.
As part of an integrated team of health and social care professionals, you will need to work flexibly in a busy environment, acting as a conduit ensuring that information on Third Sector provision is readily accessible to relevant professionals coordinating service provision to meet identified needs.

JobPurpose
The Community Liaison Officer will work proactively as part of the Community Resource Team (CRT), providing direct support and information to older people and their families in their own homes. The role focuses on empowering individuals to maintain independence, improve wellbeing and resolve specific housing, social and financial needs:
- On their discharge from hospital
- To prevent avoidable hospital admissions
- To help choose an appropriate care home
They will work in partnership with C3SC and GVS to ensure that a range of voluntary sector health and social care services are pulled together in response to the identified needs of older people who have been referred to the CRT.
This job description outlines the key performance indicators of, and output required from the Community Liaison Officer. It is not a definitive list, and the role may well change and evolve over time.
Main Duties and Responsibilities
- Provide direct support with social care tasks and offer emotional support to individuals who may be vulnerable, unwell or distressed.
- Empowering Clients to make informed choices and access the services they need.
- Build good working relationships and work closely with health and social care professionals in the CRT by responding to referrals and reporting back on Clients progress.
- Attending allocation and (where appropriate) weekly multi-disciplinary meetings.
- Encourage Clients to accept services and make informed decisions.
- Work in partnership with C3SC and GVS to raise awareness of voluntary sector services and contribute to the production of voluntary sector information resources, where appropriate.
- Provide information and liaise with a wide range of external services to ensure older peoples needs are met.
- Support applications for housing. Grants and financial entitlements.
- Contribute to the monitoring and evaluation report by assisting with the compilation of service statistics and the production of case studies as requested by the Service Manager.
- Inputting the above into the PARIS database.
- Coordinate and action referrals from the Community Resource Team and put together packages of the most appropriate voluntary sector support for older people in response to identified need.
Job Description
- Work within clearly defined working practices and organisational policy and procedure as well as work within the University Health Boards honorary contract.
- Work with the Service manager and colleagues to explore and trial any new service developments particularly in relation to the contribution of the role to the CRT.
- Contribute to changes in working practice for own work area through supervision and learn meetings.
- Responsible for submitting money given as donations to the finance team.
- Be responsible for handling Clients money for shopping and paying bills, ensuring accurate records are kept.
- Awareness of how budgetary controls and expenditure impacts the team, i.e. the effectiveness of fuel costs.
- Keeping up to date with learning and development, ensuring current legislation, codes of practice and guidance are followed.
- Take part in staff meetings as and when required.
- To be responsible for your own health and safety and the health and safety of others.
- To ensure you follow the company’s policies and processes, in particular equal opportunities, discrimination and safeguarding.
- Undertake any other reasonable duties deemed appropriate by your manager.
Person Specification
- Ability to demonstrate excellent English language skills
- Commitment to undertake/participate in appropriate training
- Good understanding of health and social care skills
- Working collaboratively with relevant health and social care staff in both statutory and third sector
- Working in a hospital or community setting
- Experience of managing caseloads
- Experience of working unsupervised whilst being a good team player
- Experience of organizing and prioritising own workload
- Experience of using a person-centred approach to guide work
- Experience of supporting individuals with housing, financial entitlements or grant applications
- Experience of producing statistical information, reports and patient stories
- Ability to empathise and communicate sensitively with older people
- Ability to initiate, plan and assist service users in gaining access to services in the community
- Ability to provide emotional support and communicate effectively with individuals in distress or with complex needs
- Driving licence and access to a car, and be willing to travel throughout the area
- Good IT skills with knowledge of Microsoft Office 365
- Flexible approach to work and a can do attitude.
