As a Personal Adviser in the Leaving Care Team, you'll play a vital role in supporting care-experienced young people aged 16 to 21, and up to 25 where required. Acting as the lead professional and case manager, you'll provide statutory support, guidance, and practical assistance to help young people build independent living skills and transition confidently into adulthood.
Key Responsibilities:
- Deliver tailored advice and support to care leavers and children in need.
- Lead case management, including assessments and pathway planning beyond age 18.
- Coordinate services and refer to specialist agencies for complex needs.
- Identify and report safeguarding concerns to appropriate professionals.
- Supervise family contact and contribute to court proceedings when necessary.
- Prepare assessments and pathway plans for eligible young people.
