Social Worker - Hospital Discharge Team Location- West Midlands Duration -12 weeks Hours per week - 37 hours Pay rate - £28.40We are currently recruiting for an experienced Social Worker to join a busy and dedicated Hospital Discharge Team in the West Midlands.
This is an exciting opportunity to work at the heart of integrated health and social care services, supporting adults to achieve safe, timely, and person-centred discharges from hospital.
You will play a key role in promoting independence, reducing delays, and ensuring individuals receive the right support at the right time to maximise their recovery and wellbeing.
The Role As a Hospital Social Worker, you will work closely with NHS colleagues, community services, individuals, and their families to facilitate effective discharge planning and prevent unnecessary hospital admissions and readmissions.
Key responsibilities include: Undertaking strengths-based assessments to identify care and support needs.
Supporting safe and timely hospital discharges in line with discharge-to-assess principles.
