You will undertake needs assessments and develop plans that respond to the identified needs of the care leaver, supporting independence and the transition to adulthood. Working with a range of agencies and professionals, you will promote good outcomes for young people and build enduring support networks based on trust and confidence.Key Responsibilities:- Manage a complex diverse caseload of young people.- Work with service users to achieve identified goals.- Undertake needs assessments and develop plans that respond to the identified needs of the care leaver.- Produce pathway plans in line with procedures and statutory guidance, ensuring they are SMART and outcome focused, and focus on achieving independence and the transition to adulthood.- Work with a range of agencies and professionals as to care planning for young people to promote good outcomes.- Work with professionals to ensure service delivery and design meets current best practice and meets the need of the community.- Build enduring support networks for the care leaver and build working relationships with the care leaver based on trust and confidence.- Understand and respond to the individual needs of the care leaver having regard to their diversity.- Encourage care leavers to participate in decision making and in the wider development of the leaving care service.- Work independently with the care leaver and their family.- Produce clear and concise reports.- Use tools and discussions with manager and team members to support with decision making.- Be accountable for own personal development in relation to the professional role and boundaries; and undertake training. #J-18808-Ljbffr
