Opus people solutions are currently recruiting for an Adult Practitioner for our client based in Telford on a temporary on-going basis until 31 August 2026.
About the Role:
You will support the delivery of statutory Adult Social Care duties in line with the Care Act, Human Rights Act, and Mental Capacity Act. This role involves working closely with individuals, families, and carers to assess needs, carry out reviews, and develop person-centred support plans that promote independence, wellbeing, and informed choice.
