This senior position sits within a dedicated Learning Disabilities and Autism team, supporting adults to live safely, independently, and with dignity. A key focus of the role is completing Care Act Reviews and ensuring individuals receive the right level of support. You'll play a central part in maximising Continuing Health Care (CHC) and other health funding, working collaboratively with health partners to secure appropriate packages of care.
Requirements:
- Qualified Social Worker with Social Work England registration, 2 years permanent experience.
- Strong experience in Learning Disabilities and/or Autism services.
- Confident in CHC processes and funding pathways.
- Skilled in Care Act assessments, reviews, and support planning.
- Able to work independently within a hybrid model.
- Car driver with access to a vehicle for community visits.
