This senior position sits within a dedicated Learning Disabilities and Autism team, supporting adults to live safely, independently, and with dignity. The role is ideal for an experienced practitioner who is confident in complex case management and committed to high-quality, person-centred practice.

A key focus of the role is completing Care Act Reviews and ensuring individuals receive the right level of support. You'll play a central part in maximising Continuing Health Care (CHC) and other health funding, working collaboratively with health partners to secure appropriate packages of care.

Requirements:

  • Qualified Social Worker with Social Work England registration, 2 years permanent experience.
  • Strong experience in Learning Disabilities and/or Autism services.
  • Confident in CHC processes and funding pathways.
  • Skilled in Care Act assessments, reviews, and support planning.
  • Able to work independently within a hybrid model.
  • Car driver with access to a vehicle for community visits.
