The Care Coordinator- Onsite plays a vital role in optimizing patients’ recovery. They conduct weekly functional assessments and collaborate with interdisciplinary teams to coordinate discharge planning, ensuring smooth transitions and informed decisions with patients and families. The role acts as a link between patients and healthcare providers to facilitate efficient care transitions.

Primary Responsibilities

  • Serve as the link between patients and healthcare personnel for efficient care transitions.
  • Perform Skilled Nursing Facility (SNF) assessments using clinical skills and CMS criteria upon admission and during stays.
  • Review outcomes and discharge plans with providers and families.
  • Complete SNF reviews and update authorizations timely.
  • Collaborate with healthcare teams for optimal discharge planning.
  • Assist patients in achieving discharge goals and resolving barriers.
  • Participate in SNF rounds, providing current information to management or medical directors.
  • Ensure appropriate referrals to health plans and community services.
  • Engage with patients and families regularly via phone or on-site visits.
  • Attend care conferences and assess ongoing appropriateness for SNF settings.
  • Review referral requests and coordinate peer reviews.
  • Support contract startup and manage caseload efficiently.
  • Maintain accurate documentation and participate in quality improvement activities.
  • Adhere to confidentiality and regulatory policies, including HIPAA.
  • Participate in cross-training and stay updated on regulations.
  • Attend meetings and promote a positive work environment.
  • Perform additional duties as assigned.

Required Qualifications

  • Active, unrestricted clinical license in the state of hire (RN, PT, OT, or Speech-Language Pathologist).
  • Minimum 5 years of clinical experience.
  • Ability to support on-site facility needs within 30 miles of home location.
  • Reside near St. George, UT.
  • Valid driver’s license and reliable transportation.

Preferred Qualifications

  • Experience with care management and utilization/resource management.
  • Proficiency with Microsoft Office applications.
  • Experience with geriatric population and home health nursing.
  • Strong attention to detail, multitasking, teamwork, and communication skills.
  • Problem-solving, conflict resolution, and independent decision-making skills.
