Overview

The role is a permanent, part-time position (30 hours per week) with Southern Housing’s Single Homeless Pathways team. You will work as part of a team across multiple locations providing support services to vulnerable residents, promoting independence within their home and the wider community. You will support daily living and social skills, health and wellbeing to people who are young or have complex needs so they can develop independent living skills, remain independent and move on to living within the wider community.

Based out of our schemes around the Island in Newport, Sandown, Shanklin and Ryde. Please note, this is a 24/7 service and you may be required to undertake sleep-in duties. A full UK driving licence is required.

Responsibilities

  • Provide support to residents and people receiving a service from us, through the principles of housing first and psychologically informed environments, as identified in their needs assessment and support plan.
  • Deliver services in a strengths-based way to promote independence, rights, choices, dignity and inclusion; summon professional assistance in emergencies when needed.
  • Work within Local Authority criteria to ensure residents have an up-to-date support plan reflecting their needs, wishes and agreed outcomes.
  • Deliver key working sessions, promote engagement, and write plans to help residents achieve their goals and outcomes.
  • Maintain and develop professional relationships with residents and stakeholders.
  • Promote life skills and social skills, encourage resident involvement, and support the Customer Involvement Framework.
  • Signpost and access specialist services to meet identified outcomes.
  • Assist with security of the resident\’s home and support maintaining a safe living environment.
  • Follow health and safety policies and procedures at all times, including lone working and infection control, and adhere to the SI&C QA Guide for Health and Safety.

Qualifications and requirements

  • A willingness and ability to train towards an NVQ Level 2 in Care.
  • Hold at least 5 GCSE passes at grade C or above (or equivalent, Grade 9-5) including Maths and English.
  • A willingness to train towards an NVQ Level 3 or equivalent relevant work-based qualification.
  • A willingness to obtain underpinning knowledge required to perform the duties of the role.
  • A good level of written and verbal communication skills; empathy, compassion and patience; ability to adapt to different working environments.
  • Full UK driving licence.
  • Be willing to undertake sleep-in duties where applicable.
