Make a real difference with Choices Homecare
Choices Homecare is looking for compassionate, reliable and adaptable care professionals to join our Rapid Response Team, supporting clients across the Kirklees region.
This is a rewarding opportunity to make a genuine difference to people when they need support most. You’ll play a vital role in helping new clients access the care they need, providing reassurance during times of change and ensuring they receive the right support from the very beginning.
If you enjoy a varied role, can adapt to changing situations and take pride in delivering high-quality, person-centred care, we’d love to hear from you.
About the role
As a Rapid Response Care Worker, no two days will necessarily be the same. You’ll support people with a range of care needs, respond to changing priorities and help establish new care services.
Working independently and alongside colleagues and other professionals, you’ll help people remain safe, comfortable and as independent as possible in their own homes.

-
Welcome and support new clients, helping them feel comfortable and reassured as their care begins.
-
Gather information about individual needs, preferences and circumstances to support the development of effective care plans.
-
Carry out care calls during the initial stages of a service, providing continuity until clients are established on their regular care rounds.
-
Complete appropriate risk assessments and identify potential risks to clients and colleagues.
-
Provide personal care, including washing, dressing, toileting and continence support.
-
Assist with medication in line with care plans and organisational procedures.
-
Support with mobility, transfers, meals, hydration and everyday household tasks where required.

-
Work collaboratively with colleagues and health and social care professionals.
-
Maintain accurate records and follow safeguarding, confidentiality, infection control and health and safety procedures.
-
Help people remain safe and independent at home, supporting safe hospital discharge where appropriate.
