About The Role
We support individuals in their own homes, assisting those with a variety of health needs including learning disabilities, mental health issues, physical disabilities and personal care needs.
Role Overview
Your responsibility is to support individuals to live healthy, happy and fulfilling lives. You will assist with daily activities, promote independence, ensure safe living environments, administer medication, and help individuals access and enjoy the local community.
Key Responsibilities
- Support individuals to live independently: encouraging and supporting them in daily activities such as meal preparation, budgeting and personal care.
- Administering medication: following medication protocols, ensuring accurate and timely administration.
- Maintaining a clean, safe and accessible home environment.
- Promoting healthy relationships with family, friends and wider community.
- Providing practical and emotional support for day‑to‑day activities such as shopping, appointments and hobbies.
- Assisting individuals in accessing community resources and activities to enhance social inclusion.
- Accompanying individuals outside the home for hospital visits, holidays or appointments when required.
- Supporting independence while respecting each individual\’s preferences and choices.
- Reporting to the Service Manager: regularly reporting on progress and ensuring care plans are followed.
Role Details
- Position type: Part time and Full time available.
- Hours: 16‑36 hours per week (bank contracts also available).
- Shift patterns: Vary – discuss during interview.
- Location: Sheffield.

- Flexibility and willingness to provide support outside the home when required.
- Alternative weekend availability.
- NVQ Level 2 or 3 in Health and Social Care (or equivalent) desirable.
- Full UK driving licence (preferred but not essential).
