Optimal Care Plus is seeking an experienced, organised, and motivated Care Coordinator to support the operational delivery of our services. This is an excellent opportunity for a skilled health and social care professional who is passionate about delivering high-quality care and supporting effective service coordination. The successful candidate must be flexible, willing to travel nationwide when required, and committed to maintaining high standards of care delivery, compliance, and documentation.
Key Responsibilities
- Coordinating and managing staff rotas and care schedules
- Ensuring continuity and quality of care delivery
- Supporting and supervising care staff
- Writing, reviewing, and updating care plans and risk assessments
- Ensuring compliance with CQC regulations, company policies, and care standards
- Supporting service user assessments and reviews
- Assisting with safeguarding, incident reporting, and investigations
- Maintaining accurate records, reports, and documentation
- Supporting the Registered Manager with operational and administrative duties
- Covering care shifts when required to maintain safe service delivery
- Travelling nationwide to provide operational support across branches and services when required
- Supporting digital systems and innovative ways of working within the service
Requirements
- Previous experience within the health and social care sector
- Experience writing and reviewing care plans
- Strong report writing and documentation skills
- IT literate with good computer skills
- Knowledge of digital systems and digital innovation within care services
- Full UK driving licence, active driver and willing to travel nationwide
- Flexibility and adaptability in supporting different branches and services
- Excellent organisational, communication, and analytical skills
- Ability to work independently and as part of a team
- Positive and professional approach to leadership and service delivery
