Registered Nurse Responsibilities
- Provide services requiring substantial and specialized nursing skill, in accordance with the plan of treatments signed by the physician and may make the initial evaluation visit to the client.
- Conduct initial and ongoing comprehensive assessments of the client's needs, including OASIS assessments at appropriate time points; evaluate and regularly re‑evaluate the nursing needs of the client, and initiate, develop, implement, and revise the client’s plan of care.
- Initiate diagnostic, preventative, and rehabilitative nursing procedures as appropriate and make referrals to other disciplines as indicated.
- Observe signs and symptoms and report to the physician and/or appropriate health professional as needed.
- Teach, supervise, and counsel the client and family regarding home nursing procedures and other care needs.
- Coordinate the total plan of care and maintain continuity of care by liaising with other health professionals; attend and initiate client care conferences as needed.
- Assign home health aide services to specific patients, supervise aides, and revise plans per Medicare Conditions of Participation.
- Supervise LPN and paraprofessionals providing services.
- Develop, prepare, and maintain individualized client care progress records accurately and timely.
- Follow all policies and procedures regarding service delivery, documentation, and care coordination, including infection control, hazardous waste management, and age‑specific behaviors.
- Participate in in‑service programs and quality improvement initiatives.
- Plan for client discharge from services.
Qualifications
- Minimum associate’s degree in nursing from an accredited program.
- Registered Nurse license recognized by the state in which the candidate is practicing.
- Minimum of one year of experience as a Registered Nurse (preferred).
- Knowledge and skills to provide care to and communicate with the adult and geriatric populations.
- Knowledge of growth and development principles over the life span.
- Ability to assess data reflecting the client’s status and interpret appropriate information for age‑specific needs.
- Computer skills sufficient to use the electronic medical record effectively.
- Valid driver’s license and current auto insurance.
- Reliable transportation.
Pay Range
$75,000 – $85,000 USD
