We have an exciting opportunity for an experienced Social Worker to join our Community Team for People with Learning Disability! This is a full‑time, permanent role with hybrid working, and a salary of £40,192 to £44,012 per annum plus an annual retention bonus of £3,000 subject to eligibility.

You will be working as a Social Worker principally to support young people in transition from Children’s to Adult Learning Disability Services. Your role will involve:
- Supporting young people with learning disabilities and their families and carers to make a positive transfer to adult services, including liaising with Children’s services and any other involved agencies.
- Carrying out assessments, undertaking care reviews and supporting care planning.
- Participating in a duty rota to deal with incidents, safeguarding alerts and information requests.
- Protecting and promoting the interests of vulnerable people.
- Undertaking safeguarding investigations, Deprivation of Liberty Safeguards assessments, capacity assessments and best interest decisions for adults with a learning disability.
What we are looking for:
- A qualified Social Worker with a qualification such as a BA, BSc Diploma or equivalent.
- Current registration with Social Work England.
- Experience of working in a health and social care environment.
- Good understanding of safeguarding protocols.
- Knowledge of relevant legislation.
- Committed to working co‑operatively as part of a team.
