We are seeking a compassionate Community Healthcare Assistant to join our Healthy Family Team, supporting children, young people, and their families across homes, schools, early years settings, clinics, and the wider community. The role involves assisting in the delivery of universal, targeted, and specialist services, providing high-quality, evidence-based care to improve health outcomes.
Main Responsibilities:
- Deliver care and support to children, young people, and families across various settings, under the supervision of qualified professionals.
- Assist with assessment, review, and progress monitoring using structured tools and feedback to qualified staff.
- Support universal, targeted, and specialist programmes, including the Healthy Child Programme.
- Run, coordinate, and support training, workshops, and group sessions.
- Build and maintain effective relationships with families, schools, and multidisciplinary teams.
- Maintain accurate records and documentation in line with organisational and national standards.
- Follow safeguarding procedures, identify risks, and participate in safeguarding supervision.
- Use clinical reasoning to implement interventions and adapt care packages.
- Ensure safe use, storage, and transportation of equipment and resources.
- Participate in professional development, supervision, appraisal, and training of students and junior staff.
- Work flexibly across services and settings, manage own caseload, and adhere to organisational policies.
Ideal Candidate:
- Good general education (minimum 4 GCSEs including Maths and English) or equivalent.
- NVQ Level 4, CACHE Level 3, BTEC in Child Care, or at least 2 years’ relevant experience with children.
- Experience working in a team and with multi-agency partners.
- Understanding of confidentiality, safeguarding, and information governance.
- Strong organisational, time management, IT, and communication skills.
- Ability to work independently and respond appropriately to children and families.
- Willingness to learn, undertake training, and adapt to changing practices.
- Diploma or Degree in Health and Social Care.
- Experience in NHS, early years, or community settings.
- Other requirements: the successful applicant will need to be a car driver and access to a car insured for work purposes.
