We’re looking for Band 5 Community Nurses to join our North Kent Community Nursing Service Swale, with a base at Sheppey Community Hospital, to help us deliver excellent holistic patient‑centred care across Sheppey, Sittingbourne and surrounding villages.

Main Responsibilities

  • Provide high‑quality, compassionate care to patients and their families.
  • Deliver clinical services, share best practice, and actively improve service delivery.
  • Collaborate with multidisciplinary teams and support colleagues through regular team meetings and one‑to‑one sessions with managers.
  • Maintain accurate patient records and utilise IT systems, including Microsoft Office applications.
  • Support patients and families with flexible, personalised care across the community.

The Ideal Candidate

  • Registered NMC/1st level Nurse.
  • Full UK driving licence with access to a vehicle for work purposes.
  • Strong communication, organisational skills and a flexible, patient‑focused approach.
  • Confidence using IT systems and working knowledge of Microsoft Outlook, Word, and Excel.
  • Ability to work independently and as part of a multidisciplinary team.
