Responsibilities
- Collaborate closely with clinicians to enhance patient care and make cost‑effective prescribing decisions.
- Run daily telephone triage clinics and assess patients clinically where indicated.
- Review and triage pathology results and contact patients as necessary.
- Provide practice‑based review and clinical assessment for patients with long‑term conditions or complex needs.
- Reconcile medicines on hospital correspondence and liaise with local pharmacies to improve outcomes.
- Lead the practice repeat prescription service and manage queries from reception staff and patients.
- Participate in home visits and immunisation initiatives to improve service quality.
Qualifications & Experience
- GPhC registered.
- Excellent verbal and written communication skills.
- Ability to work well as part of a team and willingness to develop clinically.
- Can‑do attitude and genuine desire to improve patient care.
