We have an exciting opportunity for an enthusiastic and motivated Band 4 Rehabilitation Assistant to join our new Early Supported Discharge (ESD) Stroke and Neurology Service based at Gravesham Community Hospital, Gravesend (DA11 0DG). You will work as part of a dynamic multidisciplinary team supporting patients following stroke and other neurological conditions, delivering high-quality rehabilitation in community and home-based settings. This role plays a key part in supporting patients to regain independence and achieve their rehabilitation goals following hospital discharge. You will work under the supervision of registered clinicians including physiotherapists, occupational therapists, speech and language therapists, dietitians, and nursing staff. This is a full-time position. The ESD service operates 8am – 4pm, Monday to Saturday. A full driving licence and access to a vehicle is essential, as the role involves regular community visits to patients in their own homes and travel between bases.

Main Responsibilities

  • Work as part of the multidisciplinary ESD team supporting stroke and neurological rehabilitation
  • Deliver delegated rehabilitation programmes under the supervision of registered professionals
  • Support patients with functional rehabilitation tasks in their own homes and community settings
  • Assist with implementation of physiotherapy, occupational therapy, speech and language therapy and nursing programmes
  • Undertake basic assessments and contribute to patient progress reviews under supervision (including completion of Trusted Assessor training where required)
  • Promote independence, safety, and recovery for patients in line with individual care plans
  • Support group-based rehabilitation sessions and inpatient/community interventions where required
  • Act as a first point of contact for patients, families, and professionals when appropriate
  • Assist with service administration and maintenance of equipment as required
  • Work flexibly across ESD and wider community neuro-rehabilitation services as service needs require

The Ideal Candidate

  • NVQ Level 3 in Health and Social Care or equivalent qualification
  • Experience working at Band 3 level or in a similar rehabilitation/support worker role
  • Understanding of stroke, neurological conditions, and rehabilitation principles
  • Experience working in community settings (desirable)
  • Aware of lone working in community healthcare environments
  • Understanding of confidentiality and information governance requirements
  • Ability to work independently and use initiative while following clinical guidance
  • Good communication skills when working with patients, families, and MDT colleagues
  • Ability to follow delegated rehabilitation programmes accurately and safely
  • Willingness to undertake further training and development, including Trusted Assessor training
  • Full UK driving licence with access to a vehicle (essential)
