Hours: 3 days – 22.5 hours per week

One Wight Health is seeking a compassionate, proactive, and detail-oriented individual to join our team as a Smokefree Outreach Worker. In this fully telephone-based role, you\’ll be at the forefront of supporting patients to make positive health changes and take their first steps towards a smokefree future.

Applications are clearly welcomed from anyone that is already qualified via the National Centre for Smoking Cessation and Training (NCSCT), has previously undertaken either Very Brief Advice (VBA), Making Every Contact Count (MECC), or other motivational interviewing training, but if not, you will be supported in becoming professionally certified.

Main duties of the job

Working in line with national and local guidance and cessation best practice, the post holder will work as a Stop Smoking Outreach worker across selected practices based on the Isle of Wight. They will support people across the Island who want to stop smoking or using tobacco, contributing to the reduction of smoking rates and thus improving the health outcome of the local population.

The postholder will be expected to deliver support to those recorded as having an active smoking status by their GP, and provide them with brief quitting advice, telephone support and onward referral to the core stop smoking service. Making and monitoring referrals, whilst updating patient records with the latest smoking status information.

The postholder will undertake local promotion of the outreach offer and engage with local partners, stakeholders and health professionals including GP surgeries. The postholder will be responsible for the accurate recording of patient information to improve health and update GP smoking status records.

You will work alongside other healthcare professionals and be part of embedding these services, working with key partners in Isle of Wight Council and community stop smoking services.

About us

We support our member GP practices to help them deliver the best possible care to patients on the Island. We host a team of Social Prescribers, Health and Wellbeing Coaches and Care Co-ordinators under a Primary Care Contract that support our Island practices.

We aim to do that by providing centralised services that enable GPs to enhance their offer of support to patients.

We also provide direct support to GPs and practice staff to enable them to develop, lead and manage their practices more effectively and efficiently on a day-day basis. For example, providing training and education, mentoring, recruitment support, resources, access to specialist roles and sharing best practice.

We also help bid for and secure additional funding from NHS England, where it becomes available, to help support new initiatives that help practices to meet their patients care needs and we work to support general practice/primary care, to make sure it has a voice within the wider healthcare system as it develops plans for the future care for our population.

Job description

Job responsibilities

  • Promote the health and wellbeing of people, refer to the stop smoking service, offer stop smoking brief advice, information and support.
  • Proactively contact patients recorded as smokers to verify and update their smoking status, ensuring accurate clinical coding.
  • Work with patients to change their behaviour to improve their health by stopping smoking and, on occasions, support significant others e.g. partners/ parents / family members who smoke.
  • Use motivational interviewing techniques and Very Brief Advice to encourage quit attempts and refer patients to smoking cessation support services.
  • Build strong, empathetic connections with patients over the phone, supporting behavioural change and helping reduce barriers to quitting smoking.
  • To manage the transfer of individuals between agencies and the local community stop smoking service.
  • Responsible for management of day-to-day workload.
  • Work with a wide range of patients as part of their day-to-day work, including those with communication difficulties and adapt information and support provided accordingly.
  • Provide information to people about stopping smoking, minimising harm from tobacco and health-related issues in line with best guidance and protocols. Taking into consideration the wide range of diverse needs e.g. different forms of tobacco use, English as a second language.
  • Contribute to the monitoring and evaluation of the service. This may include following up on patients discharged to community smoke free services to monitor stop smoking referrals and outcomes, then updating records to reflect the patients most recent smoking status.
  • Use initiative to communicate/propose new or innovative ways of working to improve the delivery of the service for patients.
  • Ensure administrative duties are carried out efficiently and effectively to meet the needs of the service to include accurate collection and recording of patient interaction data.
  • Use Microsoft Excel to compile monthly reports, including qualitative and anecdotal feedback.
  • Attend meetings as and when required including feeding back to funders on progress of the service.
  • Responsible for audit within own work area.
  • At all times treat patients, carers and colleagues with dignity and respect, exercising discretion and maintaining confidentiality.
  • Take every reasonable opportunity to maintain and update knowledge, competence, and skill, with a particular focus on any new developments for stop smoking.

Person Specification

Qualifications

Essential

  • High standard of written and spoken English.
  • Educated to GCSE (or equivalent) grade C / level 4 standard pass or above in English and Maths.
  • Commitment to obtain underpinning knowledge through work based learning and mandatory training.

Desirable

  • Specific training in health promotion interventions.
  • National Centre for Smoking Cessation and Training Practitioner Training / Very Brief Advice+ / Swap-to-Stop specialist modules, Motivational interviewing.
  • Knowledge of NRT used in treatment .

Skills

Essential

  • Excellent telephone communication and interpersonal skills.
  • Very good written and verbal communication skills.
  • Knowledge and experience of database management and an understanding of its importance in the effective day to day delivery of health services.
  • Ability to present data in a clear and easily understood format -both verbally and in writing, including the use of Excel.
  • Ability work on own initiative.
  • Proven team worker with ability to motivate others.
  • Aware of own strengths and development needs.
  • Flexible, enthusiastic and committed.
  • Ability to work effectively and collaboratively as part of a team.

Experience

Essential

  • Experience of working in Health and Social Care setting or community.
  • Experience of providing healthcare, healthcare advice or health improvement/promotion advice to patients or members of the public.
  • Experience of using Microsoft Office software.

Desirable

  • Experience of delivering stop smoking brief interventions.
  • Experience of providing help, behavioural support and/or counselling on a one to one or group basis.
  • Experience of working in a health improvement related field.
  • Experience using SystmOne.
  • Previous smoking cessation lived experience.
