South Wight Medical Practice is seeking an experienced and motivated Prescribing Clinical Pharmacist to join our friendly, forward-thinking multidisciplinary team. This is a patient-facing role within a rural, dispensing GP practice, with additional responsibility for acting as the practice Medicines Management Lead. The role offers the opportunity to deliver high-quality Medicines Optimisation, provide clinical leadership, and shape prescribing safety and dispensary governance across the practice.
Main Duties of the Job:
The Clinical Pharmacist Medicines Management Lead is a senior clinical role within the GP practice, providing leadership and assurance for medicines optimisation, prescribing safety and dispensing governance. The post holder will act as the practice lead for medicines management, working closely with partners, the dispensary team, and the Integrated Care Board (ICB) to ensure safe, effective and legally compliant use and supply of medicines.
Main Responsibilities:
- Deliver Structured Medication Reviews (SMRs), including polypharmacy and complex patients.
- Manage medicines for long-term conditions under agreed clinical protocols.
- Provide independent prescribing, deprescribing and medicines optimisation.
- Support repeat prescribing and medicines reconciliation following hospital discharge.
- Provide expert medicines advice to GPs, nurses and the wider MDT.
Medicines Management Lead Responsibilities:
- Act as the practice Medicines Management Lead, providing assurance to GP Partners.
- Lead prescribing safety, medicines optimisation and medicines-related clinical governance.
- Maintain oversight of repeat prescribing systems and prescription authorisation.
- Oversee medicines-related risks, audits, and significant events.
- Support CQC compliance relating to medicines and safer prescribing systems.
