Intensive Support Worker | 37 hours p/w | £28,135 | 12-month contract
The Role
As an Intensive Support Worker, you will play a key role in supporting people experiencing entrenched rough sleeping and multiple disadvantage to achieve greater stability and independence.
Working alongside the Intensive Support Lead, you will deliver flexible, trauma-informed, and person-centred support, building trusted relationships that help individuals sustain accommodation, overcome barriers, and progress towards their goals at a pace that is right for them.
Through collaborative support planning and responsive day-to-day interventions, you will make a meaningful difference while contributing to a high-quality, psychologically informed service.
Key Responsibilities
- Deliver intensive day-to-day support in line with agreed support plans, maintaining regular and flexible contact, including during periods of crisis, change, or disengagement.
- Develop, review, and update person-centred support plans that reflect each individual's needs, goals, and changing circumstances.
- Build positive, respectful, and non-judgemental relationships that encourage trust, engagement, and long-term stability.
- Support individuals to access, settle into, and sustain accommodation, providing practical assistance with tenancy responsibilities, including rent, utilities, budgeting, and maintaining a safe living environment.
- Identify early signs that an individual's stability or tenancy may be at risk and promptly raise concerns with the Intensive Support Lead.
- Recognise and respond appropriately to safeguarding concerns, completing risk assessments and developing safety and support plans in line with organisational procedures.
- Work safely within lone working and dynamic risk management frameworks, using professional judgement and escalating concerns where appropriate.
- Collaborate with internal teams and external partner agencies to deliver coordinated support and share relevant information in line with data protection and organisational policies.
- Advocate on behalf of individuals to help them access services, overcome barriers, and achieve positive outcomes.
- Promote and contribute to a safe, welcoming, and inclusive environment for all service users.
- Maintain accurate, timely, and high-quality case records, contributing to support reviews, outcome monitoring, and reporting requirements.
- Participate in regular supervision, reflective practice, and training, actively contributing to a culture of continuous learning and service improvement.
