Minehead Medical Centre is seeking an experienced Community Matron to join our primary care team. If you:
- Are a registered Nurse and a fully-qualified Independent Provider.
- Enjoy building strong, trusting relationships with patients and their families.
- Desirably, have experience in supporting older adults — including frailty, geriatrics, care homes or complex long-term conditions.
- Can deliver proactive clinical care, polypharmacy review, and anticipatory care planning.
The Community Matron will support patients of all ages with diverse and often multifaceted conditions. They will provide personalised, holistic care for people with complex health needs across our community, with a significant focus on supporting our older and often frail population in Minehead and the surrounding communities. Consequently, the Community Matron will play a central role in supporting our patients to remain independent, avoid unnecessary hospital admissions, and receive high-quality care in the place they call home. This role is ideal for someone who is passionate about proactive, frailty-focused care and making a meaningful difference to the lives of older adults.
Key Responsibilities
- Clinical Care
- Provide advanced clinical assessment, diagnosis and management of adults with complex, long-term conditions — including frail, elderly and housebound patients — in their homes, care homes and community healthcare settings.
- Deliver proactive clinical care, including falls risk assessment, polypharmacy review, and anticipatory care planning.
- Work closely with families, carers and social care teams to support older patients with complex needs.
- Leadership & Coordination
- Lead and coordinate clinical care pathways with GPs, community nursing, care homes, therapy teams, social care and voluntary organisations.
- Support the development of proactive frailty identification and early intervention within the practice and PCN.

Essential Qualifications and Experience
- Registered Nurse with valid NMC registration
- Independent Nurse Prescriber (V300)
- Significant post-registration experience in community or primary care, managing complex caseloads
- Experience of multi-agency, multidisciplinary working
- Knowledge of current NHS policy, community nursing models, and primary care priorities
