Minehead Medical Centre is seeking an experienced Community Matron to join our primary care team. Minehead Medical Centre has recently rated ‘Good’ in all areas by the CQC in its first inspection under OneMedicare’s leadership. After taking over the service in early 2024 following an ‘Inadequate’ rating of the previous provider, the new team has made rapid and significant improvements, transforming patient care, access, and leadership. We are a passionate, close-knit team committed to providing the highest standards of care in a way that truly meets the needs of our patients. The Medical Centre is a central hub in the town, and we are proud to reflect the welcoming and collaborative spirit of the wider Minehead community.
The Community Matron will be an advanced prescribing nurse practitioner, providing expert clinical care, case management, and coordination for patients with complex, long-term conditions in the community. You will play a vital role in preventing unnecessary hospital admissions, promoting self-management, and improving outcomes for patients and their families, working closely with GPs, community teams, social care, and voluntary sector partners. The postholder will be expected to work collaboratively with primary care teams, social care, voluntary sector partners, and secondary care specialists, and to act in accordance with the organisation’s values, promoting dignity, compassion, respect, and inclusivity in all aspects of care.
Key Responsibilities
- Lead care for a defined cohort of high-risk patients across the community, care homes, and practice settings.
- Provide advanced assessment, diagnosis, and prescribing where qualified.
- Support, mentor, and educate colleagues within the practice and wider primary care team.
- Ensure care is safe, high-quality, and compliant with professional and organisational standards.
Essential Qualifications and Experience
- Registered Nurse with valid NMC registration
- Independent Nurse Prescriber (V300)
- Significant post-registration experience in community or primary care, managing complex caseloads
- Experience of multi-agency, multidisciplinary working
- Knowledge of current NHS policy, community nursing models, and primary care priorities
