Location: Peripatetic across Lewisham (with Lambeth cover on Thursdays)

Hours: Part-time – 28 hours per week (7 sessions)

One Health Lewisham (OHL) is an innovative integrated community healthcare provider delivering high-quality services across South East London. Working closely with GP practices, Primary Care Networks and system partners, we develop and deliver integrated services that improve patient outcomes and support the wider NHS.

We are looking for an experienced and passionate Clinical Lead to provide strategic and clinical leadership for our Enhanced Care Home Service. This is an exciting opportunity to influence the future of care for older adults by leading service improvement, championing high-quality, person-centred care, and working collaboratively across health and social care.

About the role:

  • Provide visible clinical leadership across care home services in Lewisham, supporting the delivery of the Enhanced Health in Care Homes (EHCH) framework and ensuring residents receive safe, effective and compassionate care.
  • Work alongside GPs, nurses, pharmacists, allied health professionals, care home teams, community services and local authority partners to lead quality improvement initiatives, drive innovation, and support integrated models of care that improve outcomes and reduce avoidable hospital admissions.
  • Influence strategy while remaining close to clinical practice and service delivery.

Key responsibilities:

  • Provide strategic and operational clinical leadership for care home healthcare services.
  • Lead clinical governance, patient safety and quality improvement programmes.
  • Support the implementation of the Enhanced Health in Care Homes framework.
  • Chair multidisciplinary team (MDT) meetings and foster collaborative working across organisations.
  • Develop innovative, integrated models of care for older adults living in care homes.
  • Support improvements in frailty, dementia, end-of-life care and medicines optimisation.
  • Monitor service performance, analyse data and present findings to senior leaders.
  • Develop and deliver education and training programmes for healthcare professionals and care home staff.
  • Build strong relationships with residents, families, carers and partner organisations.

About you:

We're looking for someone who combines strong clinical expertise with outstanding leadership skills. You'll be:

  • A registered healthcare professional (Nurse, GP, Pharmacist, Allied Health Professional or equivalent) with current professional registration.
  • An experienced senior clinician with a background in older adults, frailty, long-term conditions or care home services.
  • An inspiring leader with experience managing multidisciplinary teams.
  • Skilled in clinical governance, quality improvement and service transformation.
  • An excellent communicator who can build productive relationships across organisational boundaries.
  • Passionate about improving outcomes and delivering person-centred care.
  • A Master's degree (or equivalent experience), evidence of continuing professional development and leadership training are desirable.
  • Independent prescribing (where applicable) and experience working within the Lewisham health economy would also be advantageous.
