About Us
One Health Lewisham (OHL) is an innovative, fast‑growing integrated community provider delivering high‑quality, patient‑centred services across South‑East London. We work closely with GP practices and Primary Care Networks (PCNs) to transform care pathways and improve outcomes for some of the most complex patients in the system. Our services span urgent care, home visiting, population health, digital transformation, and pioneering programmes such as Long Covid support and weight management initiatives.
Responsibilities
- Deliver high‑quality, patient‑centred care to at least eight housebound frailty patients per day in their own homes.
- Independently assess, diagnose, and manage patients with complex and undifferentiated conditions.
- Develop and implement treatment plans, including prescribing where appropriate.
- Work collaboratively with multidisciplinary teams to ensure seamless patient care.
- Provide clinical leadership and support to colleagues.
- Contribute to service development, audits, and quality improvement initiatives.
Qualifications
- Registered Nurse with full NMC registration.
- MSc in Advanced Clinical Practice (or equivalent).
- Experience working in General Practice (minimum 1 year).
- Strong clinical assessment, diagnostic, and decision‑making skills.
- Ability to work autonomously and lead clinically.
- Experience with EMIS clinical system.
- Excellent communication and interpersonal skills.
- Car owner with business insurance cover and a full clean driving licence – driving to patient homes mandatory.
- Background in leadership, service development, or quality improvement.
- Experience working within community or integrated care settings.
