Role Details

We’re looking for a passionate and proactive Primary Care Mental Health Nurse, to join our practice delivering high-quality care to our patients. This is a fantastic opportunity to contribute to a forward-thinking healthcare organisation while working in a supportive and inclusive environment.

As a Primary Care Mental Health Nurse, you’ll act as a vital bridge between primary care and specialist services. You’ll empower patients on their recovery journey through early intervention, extended consultations, and holistic assessments. Your role is key in preventing deterioration, promoting independence, and supporting mental and overall well‑being.

Main Duties

  • Provide early intervention for mental health concerns in the community
  • Support patients with issues such as stress, anxiety, depression, bereavement, and postnatal mental health
  • Manage risk and provide support around self‑harm or suicidal thoughts
  • Work closely with GPs and the wider primary care team
  • Signpost or refer patients to appropriate services, resources, and community support
  • Contribute to a holistic, patient‑focused approach to care

Role Requirements

  • Registered Mental Health Nurse (RMN) or equivalent qualification
  • Experience working with mild to moderate mental health conditions
  • Understanding of primary care and integrated mental health servicesStrong interpersonal and communication skills
  • Ability to work independently and as part of a multidisciplinary team
  • Commitment to person‑centred inclusive care
