As a Social Worker with our client, you will play a vital role in improving the lives of adults in the community through dedicated support and collaborative initiatives. The Social Worker will conduct assessments and develop personalized care plans to address individual needs and improve quality of life. Building strong relationships with individuals, families, and carers is essential to provide holistic support and reduce risk factors.
As a Social Worker, you will participate actively in interdisciplinary teams, collaborating with external resources to enhance community-based services. Additionally, you will develop and implement community interventions and group activities to promote social engagement and well-being. Contributing to forums and initiatives focused on areas such as dementia, disability, mental health, and end of life care will be part of your role as a Social Worker. Do you have?
- Social work qualification and registration with Social Work England.
- Experience working with diverse populations, including those facing Mental Health, Disability, Dementia, Isolation, and End of Life challenges.
- Demonstrated experience in community or group work settings.
- Experience providing social care to adults.
- Knowledge and experience of Safeguarding, Care Act (2014), and Mental Capacity Act (2005).
- Proficiency in social work interventions for individuals, families, groups, and communities facing complex issues.
- Driving license.
