We are recruiting a Care Manager for our client, a reputable care organization dedicated to supporting individuals with complex needs. As a Care Manager, you will oversee a team providing supported accommodation to six female residents, aged 18-40, with mental health needs.
Key responsibilities include:
- Managing daily care operations
- Scheduling shifts
- Providing supervision and on-the-job training
- Ensuring person-centered care is delivered
- Conducting regular appraisals
- Managing staff performance
- Fostering a positive, well-trained team
- Leading risk assessments
- Ensuring care plans are regularly reviewed
- Coordinating multi-disciplinary meetings to support residents' needs
