You will manage an allocated caseload of families with complex needs, chair multi-agency meetings, and coordinate coordinated support plans to improve outcomes for children and families.
You will use strength-based approaches, conduct holistic assessments in partnership with families, and develop bespoke plans that address risks and needs with partner agencies to ensure timely, coordinated support.
