The Mio Care Group is recruiting a Transfer of Care Officer to support the Transfer of Care Hub at Royal Oldham Hospital, Oldham.
The role involves triaging referrals, coordinating discharge planning, and arranging appropriate community or bed-based placements after acute care.
You will work with a multi-disciplinary team, seven days a week, to promote independence while ensuring timely, safe discharges in line with CQC and Mio Care policies.
