Job Summary
Old Swan Health Centre Group Practice is seeking an experienced and motivated Salaried GP to join our friendly, forward thinking team.
We care for a diverse patient population of approximately 9300 people and are committed to delivering high quality, patient centred healthcare.
We are looking for a dedicated clinician to provide 5 clinicalsessions per week under the ARRS scheme, working collaboratively within our multidisciplinary team.
The successful candidate will play a key role in shaping the continued development of the practice and contributing to an environment that values innovation, teamwork, and excellent patient care.

Main duties of the job
- Clinical Care Delivery
- Conduct routine clinical sessions
- Provide face-to-face and telephone consultations
- Carry out home visits as needed
- Participate in telephone triage, on-call, and enhanced rotas

We are also a training practice and have 3 Registrars and F2 doctor.

Job responsibilities
Job Description
The post-holder will manage a wide range of health needs in a primary care setting, delivering high standards of care to both registered and temporary patients.
- Clinical Responsibilities
- Conduct surgery consultations, telephone consultations, home visits, and telephone triage.
- Review and sign repeat prescriptions, pathology results, and clinical correspondence.
- Make autonomous decisions for self-referred and referred patients.
- Assess undiagnosed health concerns and screen for disease risk factors.
- Develop care plans in line with Practice protocols.
- Provide counselling, health education, and lifestyle advice.
- Admit/discharge patients and refer to external care providers.
- Maintain clear, contemporaneous consultation notes.
- Review and act upon pathology and radiology results.
- Review and act on hospital letter, documents etc when required
- Collect data for audits and issue computer-generated prescriptions.
- Prescribe in accordance with the Practice formulary.
- Organisational Responsibilities
- Comply with Practice policies (e. g., prescribing, confidentiality, data protection, health & safety).
- Commit to continuous learning and evidence-based practice.
- Participate in audits and contribute to clinical standards.
- Support development of electronic patient records.
- Attend relevant training and events.
- Service Implementation
- Apply and uphold Practice policies and standards.
- Collaborate with team members to implement guidelines.
- Participate in audits and quality improvement.
- Work closely with internal and external providers (e. g., care homes, district nurses, hospital teams).
- GP Workload Contributions
- Participate in On-Call and Enhanced Access surgeries on a rota basis.
- Undertake telephone triage as needed.
- Share paperwork and administrative duties proportionally.
- Manage routine surgery allocations and contribute to overall Practice responsibilities.
- Confidentiality
- Respect patient privacy and confidentiality at all times.
- Handle sensitive information with discretion and in line with Practice policies.
- Share confidential data only with authorised personnel and in accordance with data protection regulations.
