We are looking for a compassionate and motivated Practice Nurse with an interest in frailty to join our friendly and supportive primary care team. This exciting opportunity is for an experienced nurse passionate about improving outcomes for older adults and helping patients maintain independence, wellbeing, and quality of life within the community.
Main responsibilities:
- Chronic disease management
- Health promotion and preventative care
- Frailty assessments and care planning
- Supporting care home residents and housebound patients
- Medication reviews and monitoring
- Vaccinations, wound care and routine nursing duties
- Work closely with GPs, pharmacists, paramedics, social prescribers and community teams
Qualifications:
- Registered nurse with current NMC registration
- Experience in primary care or community nursing
- Interest in frailty, elderly care or complex care management
- Excellent communication and teamwork skills
- Compassionate, proactive and patient‑centred
- Experience with long‑term condition management and care planning
- Additional qualifications in frailty, gerontology or long‑term condition management advantageous

About the practice:
We currently care for more than 13,000 patients in a semi‑rural setting, run by 3 GP partners and 7 salaried GPs, and consistently achieve high levels of QOF targets. We are a forward‑thinking practice committed to delivering excellent patient care and supporting the wellbeing of our whole team.
