We are looking for an experienced Personal Adviser (Leaving Care Worker) to join a dedicated team supporting care-experienced young people aged 16–25 as they transition into independent adulthood.

Location: North Hykeham, Lincoln (LN6 8QL)

Rate: Β£25.00 per hour

Contract: 3 months+ (likely to be extended)

Hours: 37 hours per week, Monday to Friday, 9:00am – 5:00pm

Start Date: ASAP

The Role: As a Personal Adviser, you will provide information, advice, guidance and practical support to young people leaving care. You'll work closely with them to develop Pathway Plans that focus on helping them build the skills and confidence needed for independent living.

Your responsibilities will include:

  • Building positive and trusting relationships with young people
  • Developing and reviewing Pathway Plans
  • Supporting young people with accommodation, education, employment, training, finances, health and wellbeing
  • Coordinating support with partner agencies and professionals
  • Carrying out statutory face-to-face visits
  • Managing risk and safeguarding responsibilities
  • Supporting young people through periods of crisis
  • Maintaining accurate case records and reports

About You:

  • Experience supporting young people aged 16–25 within Leaving Care, Children's Services, Youth Support or a similar setting
  • Strong knowledge of safeguarding, risk assessment and child development
  • Experience working with vulnerable young people
  • Experience working with multi-agency professionals
  • The ability to work independently and manage a varied caseload
  • A full UK driving licence and access to your own vehicle (essential)
