An exciting opportunity has arisen for an enthusiastic and self-motivated Occupational Therapist to join our Primary Care Home Hub (PCH) based at Hamilton House in Lowestoft. The Primary Care Home is a multidisciplinary team of Nurses, Therapists, Assistant Practitioners, Health Care Assistants, and Rehabilitation Support Workers delivering nursing, therapy and reablement/rehab services to adults in their own homes.

You will be completing occupational therapy assessments of patients with diverse presentations to provide a diagnosis of functional status to develop and deliver individualised person-centred therapy plans. You will hold responsibility for your own caseload, provide support and encouragement to patients in order to promote independence, respond to patient and family needs, prevent unnecessary hospital admission and support end of life care at home.

You will have access to direct supervision. Supervision takes the form of individual and group meetings, clinical reasoning/case study sessions and peer review.

Main duties of the job:

  • To provide a high standard of occupational therapy within the integrated community services as part of a multi-disciplinary team to patients within a range of domiciliary settings.
  • To provide clinical case management for a caseload of patients who require occupational therapy.
  • Participate in rapid assessment for patients who present with a variety of acute, chronic, and age-related conditions.
  • To work autonomously at a level appropriate to clinical competence and within scope of professional practice.
  • To supervise delegation of work to the Therapy Assistant Practitioners.

The service uses SystmOne as a clinical IT system to support mobile working. The service operates 7 days a week from 7:00 to 20:30 hours. Applicants for this post will be required to work between Monday to Friday, flexibility of start and finish times will be considered. At times you may be required to work with another of the 4 PCH's from a different location to cover staff sickness or capacity pressures.

Person Specification:

Personal Attributes:

  • Physical and mental capacity to deal sensitively with patients who may have high levels of anxiety and aggression.
  • Capacity to concentrate for considerable periods of time during assessment and management of patients.

Experience:

  • Essential: Evidence of clinical placements in appropriate areas as a student or previous working.
  • Desirable: Previous experience as a Band 5 Occupational Therapist.

Qualifications:

  • Essential: Diploma / Degree or equivalent in Occupational Therapy; Registration with HCPC.
  • Desirable: Evidence of CPD folder and lifelong learning; Evidence of Preceptorship.

Skills and Knowledge:

  • Essential: Knowledge of occupational therapy in relevant area; Forward thinking; Excellent interpersonal skills; Self-directed learning; Ability to work independently; Ability to pass on skills/knowledge to others within both formal and informal environments.
  • Desirable: Critical Appraisal; Audit; Research; Presentation skills.
