Are you passionate about supporting patients in regaining independence or receiving compassionate end‑of‑life care? Do you want to grow your skills in a collaborative, multidisciplinary environment? If so, we’d love to hear from you. Our nurse and therapist led Intermediate Care Unit offers a rewarding setting, focused on rehabilitation and reablement, with six Specialist Palliative Care inpatient beds. We use a team nursing and therapy approach, delivering person‑centred care, with support workers working directly alongside qualified nurses or therapists in a collaborative environment.

This Band 3 role is ideal for experienced support staff. If you are less experienced, you can start as a Band 2 Rehabilitation Support Worker or Healthcare Assistant, with a clear development pathway to progress to Band 3 through training and supervised practice. We’re looking for committed, compassionate individuals who want to make a meaningful impact every day.

Please note: This is a 24/7 service and requires availability for nights, weekends, and bank holidays.

Responsibilities
  • Play an active role in helping patients regain independence and providing comfort in the final stages of life.
  • Deliver delegated care and clinical interventions in line with your training and competencies.
  • Pressure ulcer prevention and management.
  • Wound care.
  • Catheter care and bladder scanning.
  • Venepuncture.
  • Recording clinical observations, including NEWS2 and blood glucose testing.
  • Supporting physiotherapy and occupational therapy programmes within your scope of practice.
  • Work as part of the multidisciplinary team to deliver proactive interventions including reablement and conduct therapy exercises.
  • Support therapy staff with access/discharge home visits.
  • Support therapy staff to deliver various activities to aid functional ability e.g. group exercises, games, and puzzles to improve manual dexterity and memory.
  • Work in partnership with senior members of the team and ensure the care provided is of high quality.
  • Work with patients towards self‑care and independence.
  • Work to support families and carers.
  • Be aware of the effects of illness on the family.
  • Enable individuals and their families to express their feelings and preferences and take appropriate action.
  • Work as part of the multidisciplinary team to deliver supportive care to patients approaching end of life.
Qualifications & Person Specification
  • Essential Skills and Knowledge: Additional skills as per Job Description or ability to successfully undertake the required training to achieve competency. Good interpersonal skills. Good written and verbal communication skills. Good ability to listen. Common sense and practical problem solving approach. Use of email and basic information technology skills.
  • Essential Qualifications: Level 2/3 NVQ Health related or equivalent experience in Health or Social Care. Care Certificate (Completion is mandatory once in post). Education to GCSE level Grade 5 in English and Mathematics or equivalent.
  • Desirable Qualifications: Training in communication skills. Evidence of personal development.
  • Essential Experience: Previous relevant experience of working in health or social care environment. Awareness of the needs of older people. Ability to work without direct supervision. Ability to prioritise workload.
  • Desirable Experience: Previous experience of working in a community hospital setting. Understanding of Intermediate Care Services.
  • Essential Personal Attributes: Team player. Approachable. Reliable. Flexible, and prepared to work all shift patterns (days/nights/weekends and bank holidays). Willingness and ability to work across different sites and travel to alternative sites and across the community as required.
