We are looking for a new nurse leader to join our exceptional clinical team, based out of our Newton Abbot office. This is a fantastic opportunity to help lead our top-rated and fast growing Complex Care service at a pivotal time in our company journey.
Role & Responsibilities
- You will conduct in-depth health assessments, develop and implement bespoke care plans, and coordinate care with a multi-disciplinary team.
- You'll empower patients and their families, monitor outcomes, and drive continuous improvement in care delivery.
- This leadership role offers the chance to make a tangible difference in complex care.
- Main tasks include:
- Conducting in-depth health assessments.
- Developing and implementing bespoke care plans.
- Working closely with the clinical team and Care Team Managers to lead the very best care and outcomes for our service users.
- Coordinating care with GPs, therapists, and social workers.
- Ensuring the safety and effectiveness of care packages.
- Contributing to local business growth and commissioning initiatives.
- Empowering patients and their families.
- Monitoring outcomes and driving continuous improvement.
Requirements
- NMC Registered Nurse: Active PIN in Learning Disabilities Nursing or Children's Nursing preferred.
- Proven experience in complex care or related nursing roles.
- Proactive, compassionate, highly organised, and a natural leader.
- Excellent communication and interpersonal skills.
- Strong analytical and problem-solving abilities.
- Ability to work independently and as part of a team.
- Commitment to providing high-quality patient care.
- Experience with care planning and coordination.
- Knowledge of relevant legislation and guidelines.
- Ability to manage time effectively and prioritise tasks.
