We are looking for a new nurse leader to join our exceptional clinical team, based out of our Newton Abbot office. This is a fantastic opportunity to help lead our top-rated and fast growing Complex Care service at a pivotal time in our company journey.

Role & Responsibilities

  • You will conduct in-depth health assessments, develop and implement bespoke care plans, and coordinate care with a multi-disciplinary team.
  • You'll empower patients and their families, monitor outcomes, and drive continuous improvement in care delivery.
  • This leadership role offers the chance to make a tangible difference in complex care.
  • Main tasks include:
  • Conducting in-depth health assessments.
  • Developing and implementing bespoke care plans.
  • Working closely with the clinical team and Care Team Managers to lead the very best care and outcomes for our service users.
  • Coordinating care with GPs, therapists, and social workers.
  • Ensuring the safety and effectiveness of care packages.
  • Contributing to local business growth and commissioning initiatives.
  • Empowering patients and their families.
  • Monitoring outcomes and driving continuous improvement.

Requirements

  • NMC Registered Nurse: Active PIN in Learning Disabilities Nursing or Children's Nursing preferred.
  • Proven experience in complex care or related nursing roles.
  • Proactive, compassionate, highly organised, and a natural leader.
  • Excellent communication and interpersonal skills.
  • Strong analytical and problem-solving abilities.
  • Ability to work independently and as part of a team.
  • Commitment to providing high-quality patient care.
  • Experience with care planning and coordination.
  • Knowledge of relevant legislation and guidelines.
  • Ability to manage time effectively and prioritise tasks.
