As a Carer Adviser you will work directly with unpaid adult carers to provide high‑quality information, advice and practical support. You will help carers access services, engage with communities and maintain their caregiving role. By completing statutory Carer Assessments, developing personalised support plans and collaborating with health, social care and voluntary partners, you will support carers’ mental, physical, emotional and economic wellbeing, reduce crises and improve access to local resources.
Role Description
- Carer Support: Meet carers in person, by telephone and online to build trusting relationships. Provide clear information, advice and support. Complete statutory Carer Assessments and develop support plans reflecting carers’ goals. Identify and address carers’ needs, promote wellbeing, and respond to changing circumstances. Empower carers to maintain positive lifestyles, provide ongoing support and target additional help for those with significant needs or experiencing isolation. Support carers balancing caregiving with paid employment or volunteering.
- Locality based working: Embed within a defined locality, understand local services, build a proactive presence, attend carer support groups, raise awareness of unpaid caring and contribute to service improvement initiatives such as Carers Week.
- Partnership working: Collaborate with health, social care and voluntary partners (GPs, social prescribers, adult social care) to provide a holistic person‑centred approach and support carers’ independence.

Qualifications, Education and Training
- Essential: GCSE grades 4‑9 (Maths & English) or equivalent.
- Desirable: Relevant qualifications in health, social care, information and advice.
Experience
- Essential: Working with people using a person‑centred, strengths‑based approach; identifying individual issues and providing relevant information; working collaboratively with statutory and voluntary agencies.
- Desirable: Experience in health or adult social care, community outreach or development, charity environment, working with volunteers, maintaining a database.
Relevant Knowledge and Skills
- Essential: Excellent oral and written communication; ability to relate to people at all levels; IT skills including Microsoft Excel & Word, email, database and accurate record keeping; knowledge of safeguarding and confidentiality procedures.
