Job Purpose
The role includes working within an allocated zone, responsible for evaluating and assessing new client packages, developing care plans, carrying out risk assessments, conducting reviews of existing packages, liaising with Commissioners and clients, evaluating care progress and recommending changes to the care plan if required. Mentoring and leading a team of Care Support Workers. Carrying out staff observations and supervisions.
Role Responsibilities
- Carrying out assessments of all new packages.
- Meet and greet with potential private clients and/or their families.
- Re-assess existing packages, for example when a client is discharged from hospital with a change in their needs.
- Carrying out required risk assessments to ensure delivery of safe care.
- Updating care plans, tasks, risk assessments, medication information and other care related documents found within the client’s electronic care plan and care folder, as appropriate.
- Undertaking client reviews under the direction of the Care Coordinator and Deputy Manager.
- Undertaking staff observations, competency assessment and/or supervisions under the direction of the Manager.
- Liaise with other health and social care professionals.
- Undertake care work, when required to meet the needs of the business.
- Working hours to include a minimum of one evening and an alternate weekend.
- Be part of the on-call rota, ready to cover any outstanding care visits at very short notice.
- Available to attend Team Leader Meetings on a monthly basis.
Core Responsibilities
- To maintain your uniform and all company property issued to you to a standard compatible with Candlelight Care’s expectations.
- Refrain from wearing jewellery, false nails or nail varnish whilst on duty, leading by example and promoting good infection control procedures.
- You are required to conduct yourself at all times in a cordial, respectful and courteous manner, even in difficult situations.
- Maintain good working relationships with your Office Team, other care and support workers, clients and their families/representatives.
Qualifications and Experience
- Previous experience within a care role.
- Good working knowledge of care and support work.
- Understanding and respecting the need for confidentiality.
- Good working knowledge of Health & Safety.
- Good working knowledge of personal rights and responsibilities.
- Awareness and importance of preserving clients' dignity.
- High level of literacy skills (you will be required to read care plans and record simple details).
- Diploma level 2 in Health & Social Care.
- Ability to work without constant supervision.
- Caring and empathetic approach.
- Reliable and trustworthy.
- Clean and tidy appearance.
- Ability to manage your own time.
- Good team player, who is understanding and supportive of colleagues.
- Good communication skills (to understand clients and make yourself clearly understood).
Working Hours and Availability
Full time/Part time opportunities. Full driving licence and access to own transport is essential. Minimum one evening and an alternate weekend. Be part of the on-call rota, ready to cover visits at very short notice. Available to attend Team Leader Meetings on a monthly basis.
