The predicted salary is between 18 - 18 £ per hour.

Location: Walsall Council

Rate of pay: £18ph PAYE

Working Pattern: 37 hours

Working Structure: Hybrid - 2 mandatory days in the office (Walsall work on a 3 monthly rotation across all areas of the team, so for 3 months in every 12 workers may be based at the acute setting, this will require office based 5 days a week for that period only)

Duration: 3 Months

Essential requirements: Enhanced DBS (Adults & Childrens Barred List) & Driver

About the Role

You will work as part of a multi-skilled team responsible for meeting the assessment, care management and support needs of vulnerable adults and their carers. The role focuses on enabling individuals to exercise choice, maintain independence and access appropriate community resources, while promoting health, wellbeing, and person-centred outcomes. You will contribute to assessments, support planning and care management activities in line with legislation, including the Mental Capacity Act, and Council policies.

What We're Looking For:

  • Experience or strong understanding of working with vulnerable adults and carers within a health or social care setting
  • Knowledge of person-centred approaches, self-directed support and promoting independence
  • Ability to communicate effectively with service users, families, carers and professionals
  • Understanding of safeguarding, equality, diversity and anti-discriminatory practice
  • Awareness of the Mental Capacity Act and its application in practice
  • Strong organisational skills, with the ability to manage casework and maintain accurate records
  • Ability to work collaboratively within a multi-disciplinary team and alongside community agencies
  • Competence in using IT systems for record-keeping and communication
  • Flexible, proactive approach with a commitment to professional development

Key Responsibilities:

  • Contribute to assessments and support planning to enable individuals and carers to make informed decisions and access appropriate services
  • Facilitate the assessment of needs for vulnerable adults and carers, including contributing to more complex or specialist assessments
  • Support individuals to maintain health, wellbeing, independence, choice and control, considering mental capacity at all times
  • Undertake allocated care management duties in line with policies and procedures
  • Build effective, person-centred relationships with service users and their families/carers
  • Work collaboratively with multi-disciplinary teams and partner agencies to coordinate support and access to community resources
  • Identify needs for assistive equipment and arrange provision, including referrals for specialist assessments where appropriate (following training)
