We are looking for an experienced, talented, and compassionate Hospice Social Worker for a full-time position.
What will you be doing:
The Hospice Social Worker will work as a part of a home-based interdisciplinary team providing counseling and targeted interventions for a population of homebound patients and their caregivers who are receiving Palliative or Hospice services through NVNA and Hospice. You will conduct an initial psychosocial assessment of the patient/caregiver contributing to the Agency's plan of care. The Hospice Social Worker strives for optimal patient outcomes with improved functioning and an increased ability to self-manage their bio-psych-social challenges.
What we’re looking for in you:
- Current MA license in good standing
- Bachelor's degree in counseling or closely related field
- Master of social work from an accredited program (MSW)
- 3+ years of social work experience
- Hospice work a plus
- Previous community health and medical health experience preferred
- In-depth knowledge of social service skills and resources
- Good communication skills, both verbal and written
- Ability to harmoniously interact with patients/clients, families, and colleagues
- Handle pressure in various work environments and work in the field
- Ability to work independently with minimum supervision
- Excellent organizational, flexibility, and time management skills
- Keeping up to date with education, policies, and procedures
- Ability to cover the full South Shore field and in the future focus on one central area
- Flexibility for on-call holiday and weekend rotation
