We are looking for a talented and compassionate Hospice Triage LPN or RN to work weekends.

This position is responsible for managing all aspects of patient calls ensuring that all customers are treated with the highest level of customer service. Provides support to Hospice Clinical Manager and clinical field staff. Shift is Friday – Sunday 8:00am – 8:00pm.

What will you be doing:

  • Facilitate the collection of information required in triaging patient inquiries
  • Provide support to the clinical staff in the field by coordinating phone calls to physicians, patients, and families regarding medication, treatments and management of symptoms
  • Work closely with Hospice Liaison Team in the coordination of referrals
  • Ensure that all referrals being processed for admission comply with the Medicare Conditions of Participation
  • Demonstrate exceptional customer service to patients, families, referral sources, co-workers and other customers
  • Assure compliance with all federal and state laws as well as agency compliance policy in the prevention of fraud and abuse

What are we looking for in you:

  • LPN or RN license required (in good standing)
  • 2+ years in an office setting, 1+ year in home health preferred
  • Hospice Experience is mandatory
  • Strong organizational skills
  • Ability to multi-task in a fast-paced environment
  • Strong interpersonal skills
  • Able to communicate effectively and courteously, both verbal and written
  • Able to troubleshoot, problem-solve quickly and efficiently
  • Familiar with Electronic Medical Record
  • Flexibility to work weekends and local to office preferred
