What Will You Be Doing
We are looking for an experienced, talented, and compassionate RN Case Manager for a full-time position in the Hull area.
The RN Case Manager provides direct patient care in the home, conducting assessments, developing care plans, and collaborating with physicians to ensure optimal care. The role involves care coordination and identifying additional service needs.
What We’re Looking For In You
- Current Massachusetts Nursing license in good standing.
- Registered Nurse (bachelor’s degree preferred).
- 2+ years of nursing experience required.
- Strong clinical assessment skills and the ability to prioritize and triage patients.
- Exceptional communication skills and teamwork ability.
- Excellent organizational and time management skills.
- Ability to work independently and access resources for patients.
- Strong computer skills.
