Key Responsibilities

  • Administer medications, wound care, injections, and other nursing procedures as needed.
  • Offer support and guidance to patients and their families, promoting health education and disease prevention.
  • Collaborate with GPs, social workers, and other healthcare professionals to ensure holistic patient care.
  • Maintain accurate patient records and report on patient progress.
  • Conduct risk assessments and ensure that care is delivered in accordance with NHS guidelines and best practices.
  • Act as an advocate for patients, ensuring their voice is heard in decision-making.
  • Participate in multidisciplinary team meetings and case conferences.
  • Support the discharge process by coordinating care for patients transitioning from hospital to home.
  • Promote independence and self-management for patients with long-term conditions.

Qualifications and Skills

  • Registered Nurse (RN) with a valid NMC Pin.
  • Experience in community healthcare or district nursing (preferred but not essential).
  • Strong clinical skills, with the ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Compassionate, empathetic, and able to build rapport with patients from diverse backgrounds.
  • Knowledge of health and safety regulations and infection control practices.
