Key Responsibilities
- Provide high-quality nursing care to patients in their own homes and community settings, ensuring dignity and respect.
- Assess, plan, implement, and evaluate individualized care plans for patients with complex needs.
- Administer medications, wound care, injections, and other nursing procedures as needed.
- Offer support and guidance to patients and their families, promoting health education and disease prevention.
- Collaborate with GPs, social workers, and other healthcare professionals to ensure holistic patient care.
- Maintain accurate patient records and report on patient progress.
- Conduct risk assessments and ensure that care is delivered in accordance with NHS guidelines and best practices.
- Act as an advocate for patients, ensuring their voice is heard in decision-making.
- Participate in multidisciplinary team meetings and case conferences.
- Support the discharge process by coordinating care for patients transitioning from hospital to home.
- Promote independence and self-management for patients with long-term conditions.
- Provide end-of-life care, ensuring patient comfort and dignity.
Qualifications and Skills
- Registered Nurse (RN) with a valid NMC Pin.
- Experience in community healthcare or district nursing (preferred but not essential).
- Strong clinical skills, with the ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Compassionate, empathetic, and able to build rapport with patients from diverse backgrounds.
- Knowledge of health and safety regulations and infection control practices.
