Our client is seeking a dedicated and compassionate Patient Care Coordinator to play a central role in supporting patients throughout their entire healthcare journey. This is a highly patient-focused position, ideal for someone who thrives on delivering exceptional service while working closely with clinical teams.
The Role
As a Patient Care Coordinator, you will be responsible for managing and supporting each patient from their initial enquiry through to treatment and final discharge. You will act as a key point of contact, ensuring clear and effective communication between patients and their treating clinicians at every stage. Your responsibilities will include:
- Identifying patient needs
- Booking and referring patients to the appropriate consultants
- Proactively maintaining engagement through aftercare and follow-up
- Gathering feedback and providing patient support
- Managing patient workflows efficiently, allocating appropriate timelines from first contact to discharge
- Chaperoning patients during consultations
